Tofu industry workers have a high risk of experiencing low back pain. This is because the work process involves significant force, transfer and manipulation of loads, which can place excessive stress on the lower back. Non-ergonomic postures, such as bending or lifting weights in the wrong position, are often required in daily tasks, increasing stress on the muscles of the lower back. The study aimed to prepare a standardized and ready-to-use questionnaire for research on the prevention of LBP in tofu industry workers. The study used a cross sectional design. The research sample was all workers in Jambi City as many as 88 workers. The sampling technique used purposive sampling. Data collection was done by interview using a questionnaire. Testing the validity of the questionnaire was carried out using the Pearson Product Moment technique while the reliability test was used using the Chronbach Alpha technique. Instrument validity testing of 16 questions declared valid with a correlation value greater than 0.2072. While the reliability test of LBP prevention behavior question items shows the value of Alpha Chronbach> 0.60, then the instrument is declared reliable or consistent. The results of the validity and reliability test of the instrument prove that the LBP prevention behavior instrument has a validity and reliability value that meets to be used to measure LBP prevention behavior in tofu industry workers.
